A dear esteemed colleague, We are a team of doctors from All India Institute of Medical Sciences (AIIMS), New Delhi. We commonly see that our patients do not bring relevant medical documents/ papers, which affects our clinical decision making. With due approval from AIIMS Ethics committee, we are conducting this national level survey among doctors to better understand the problem and provide relevant solutions.
