A 23-year-old man presented to the ED with rapidly progressive left arm pain and erythema. He was well until the previous night when his left finger got injured during a hockey game. Overnight the finger became increasingly painful, with erythema spreading to the wrist. Examination On presentation, his body temperature was 98.1°F, HR= 64bpm, and BP= 139/85mm Hg. The physical examination revealed small blisters or abscess on his left third finger. The finger was warm, tender, erythematous streaking was seen till the elbow.
In a while, streaking progressed to the axilla. Drainage from the finger was cultured. The patient quickly underwent incision and drainage of the lesion, and cefazolin and vancomycin were initiated.
