I have two patients with similar complaints, cracking, fissuring and inflammation of the Philtrum, do not respond to emollients and moisturisers, respond only to steroids but gets better only by 40% and persists. Can anyone help with diagnosis?

Have tried correcting any sort of deficiencies also.
