I had a 58 yr patient with acute urinary retention with Gr 2 BPH on a USG KUB, prostratic parenchymal echotexture normal, capsule intact. He was already on tamsulosin and dutasteride since last 6 month. I asked for a PSA estimation, the result of which was 5.7ng/ml(>4). As this patient was already on dutasteride which may interfere with exact PSA estimation, what should be my next step?

My other query is that should not a patient with prostatic enlargement not started on Dutasteride before PSA estimation? I feel Only Tamsulosin or its equivalent should be started with. Correct me if I am wrong and give your opinions. ​ ​ ​ ​
