The inter scalene triangle is easily palpated in the neck, next to sternocleidomastoid muscle, especially in Deep Inspiration. Trunks of Brachial plexus pass through it. The Interscalene Triangle can be divided into 3 parts cephalocaudally i.e.: The upper third is at the apex of the triangle, The middle third and The lower third is nearer to the clavicle. My observation is that: For Shoulder, humerus and clavicle surgeries, give block in the upper third of the inter scalene triangle.

For Elbow, Radius, Ulna - Give block in Middle third of inter scalene triangle. For wrist and Hand surgeries- Give Interscalene block at the lower third of the triangle. Note: If ulnar sparing is observed, Block ulnar nerve at the elbow. Share your views.
