A 46-year-old female presented with malaise, sore throat, and a low-grade fever. For the last few days, she had suffered from high fever, nausea, vomiting, and chill. Her past medical history includes chronic anemia and hypothyroidism. Physical examination revealed: Temp: 98.8 F Pulse: 102 bpm BP: 100/70 No joint swelling or erythema No rashes on the body Beta-hemolytic Streptococci sp . were identified in her urine samples, and she was admitted to the hospital. She was put on antibiotic therapy until her symptoms subsided and her blood and urine cultures were negative for bacteria.
Eleven days later, she complained of bilateral knee pain and swelling. A rheumatologist was consulted, who noted large, warm effusions of both her knees and a mass in her left proximal calf.
