Adenoid cystic carcinoma (ACC) of the lacrimal gland, though rare, remains the most frequent epithelial malignancy of this gland, marked by high recurrence, metastasis, and mortality. This major review (Pal et al., Indian J Ophthalmol, 2025) analyzed 806 cases over 30 years, revealing that globe-sparing excision combined with radiotherapy offers better survival than traditional exenteration, without compromising local control . Basaloid and sclerosing histologic subtypes carried poorer prognoses, while early-stage disease (<T3) and multimodal strategies improved outcomes.
