Dear friends, First time in my 3 years of practice, I saw at least 15 cases of complicated cerebral malaria with early MODS within last 1 month, in which with aggressive IV Artesunate therapy (including Clindamycin/ Doxycycline/Azithromycin to prevent Artesunate resistance) and supportive treatment, we succeeded to save all the lives but most of them dischaged on request with Falciparum Malaria positive due to financial exhausion. On discharge most of them were prescribed either Lumifanrine Artesunae combination or oral quinine. On follow up visits, most of them were having Falciparum Malaria positive again.

Again I had to put them on Quinine with Clindamycin/ Doxycycline/Azithromycin to prevent Quinine resistance or had to extend the course of Quinine combination therapy. Would you believe that few of them are having still Falciparum Malaria positive but having no symptoms and good…
