A 2025 randomized controlled trial in Annals of Surgery compared segment 4b/5 resection and wedge resection during radical cholecystectomy for gallbladder adenocarcinoma . Among 163 patients , the 4b/5 group had longer operative time and higher blood loss , but both approaches showed similar morbidity, mortality, R0 rates, disease-free survival, and overall survival after a median 27-month follow-up. The findings suggest that the type of liver resection does not impact long-term oncologic outcomes , reinforcing a focus on surgical quality rather than extent.
