Open globe injuries are full-thickness defects of the corneal or scleral wall resulting from blunt or penetrating trauma. These injuries represent ophthalmic emergencies due to the high risk of vision loss and require prompt recognition and surgical intervention. Clinical signs may include a peaked pupil, hypotony, prolapse of intraocular contents, or a positive Seidel test. Imaging with a CT scan may aid in diagnosis when clinical evaluation is limited. Early administration of systemic antibiotics, tetanus prophylaxis, and timely surgical repair are critical to optimizing visual outcomes and preventing complications such as endophthalmitis or sympathetic ophthalmia.
