In DM, When Diabetic foot develops, the plus or minus of bone involvement is an important turning point in the decision making regarding treatment. If the bones of the foot are involved in different permutations and combinations, the pathology also differs in every patient. Very important is the development of osteomyelitis leading to fragmentation of the bones of the toes and metatarsal bones. In every patient with foot involvement in DM, the basic investigation namely X-ray of the foot is very important and unfortunately not taken at the GP level.

This leads to chronicity and so delay in improvement. Unless otherwise, there should be an effort needed on the part of the consultant to treat the bone involvement by cyclical antibiotics for three months before deciding amputation unless some other indication for amputation.
