Case presentation: A 22-year old woman was presented to the emergency room complaining of abdominal pain. She was 7 weeks and 4 days pregnant and had a normal vaginal delivery 18 months ago. Her menstrual cycle was normal, and she did not have any gynecological history. She however had a history of mild asthma and two episodes of cystitis and has a tendency of constipation. The pain started two nights ago and was localized to right iliac fossa. She has not taken any medicine to relieve the pain.

She has urinary frequency but there are no signs of dysuria or hematuria. She has slightly reduced apatite but does not feel feverish or sweaty. Examination: Normal body temperature and blood pressure, no signs of anemia. There is focal tenderness on palpitation of the right iliac fossa with slight rebound tenderness but no guarding.
