A 58 years old woman came with acute severe spasmodic pain in lower abdomen radiating in anus and vagina. H/ O passing 4-5 semisolid stools with Frank mucus. Frequent urge for urination but every time passing few drops. Also has pain in lower back region. Patient is known diabetes, gout control with medications.
Patient has fever 100 degree F with shivering in between and has nausea and vomiting. Investigations:- CBC Hb 8.4 gms, TLC 14000/ cmm, DLC : N 80%. Urine C/ E Blood ++; deposits + ve ; M / E per hpf :- pus cells many; rbc 25-30; Uric acid 4.6 ; HbA1c 6.6 ; thyroid profile normal; urea33 ; creatinine 1.2; lipid profile Tgs 385 other parameters normal; Vitamin D 13.5 ng. U/ S abdomen no abnormal observation. Advise investigation further needed and management of this case .
