I admitted a 30 years old patient on 16th May with a high fever. His TLC was 26600 and neutrophils 90. We started him on ceftriaxone+ sulbactam IV 12 hourly, amikacin 12 hourly, and paracetamol IV 6 hourly along with other support. The patient responded well and after 3 days his TLC was 12500 and even fever was almost normal.
But on the 4th day his TLC was again 16700 and fever was 103 with the same treatment. Even his ESR is 147. My question, can it be MDR pseudomonas infection, or can it be TB as his brother informed about anti coach's treatment 1 yr back. Any suggestions?
