I am running a 35 bedded hospital in a rural area of Gujarat "Shreeji Hospital". The case of penetration wound by a high-speed metal foreign body. It travelled the whole length of liver entering the right lobe and exiting the left lobe. This patient came in an emergency in shock with absent peripheral pulse and no BP,  X-ray shows the foreign body in the abdomen when tapping done it was blood, there was no time even to send the patient for USG.

A patient was almost gone but immediately we shifted the patient in OT and given incision within 20 minutes of admission. Patient's blood group was A +ve, but this group was unavailable in all nearby blood banks, so we transfused cross matched O +ve Blood.
