Primary PCI is the gold standard. Yet, it remains inaccessible to the majority of patients. 1 Delays in primary PCI are common and can result in increased mortality and morbidity. 2 Contrarily, fibrinolytic are easily available, economical, and evaluated in several clinical studies. A pharmaco-invasive approach that is first administering early fibrinolysis and then systematically performing an angiography (and a PCI if needed) is recommended.
