His complaints started a week ago when he had an episode of rhiniitis and mild fever, which progressed to cough with sputum production. The sputum was initially white colored. But since 4 days the sputum has become darker with streaks of blood accompanied with high grade fever. Since 2 days he started experiencing pain on the right side of chest which he describes as a feeling of 'heaviness'.
He is a smoker, hypertensive under regular treatment, with no history of coronary artery disease, diabetes or tuberculosis. He is manual laborer and lives in an urban slum.
