Recently, I happened to get a police memo requesting to issue a treatment certificate for an individual Mr. X, I had treated more than 3 weeks back. The concerned police was from a police station situated 200 km from my hospital (the said incident had occurred in their area). The patient I had treated presented to me with a small lacerated wound over scalp allegedly received while he happened to hit his head while he was getting out of his car. The wound was consistent with the history of injury.

I treated the wound and prescribed him the medicines. The case paper print out was handed over to the patient and a hard copy was saved to my laptop. After 3 weeks the police came asking for a treatment certificate. However, my record didn`t show anyone by the name the police mentioned in the memo and so I was unable to give them the certificate.
