A 69 yr female patient presenting with h/o fever since 1 week, hemoptysis and dyspnea since 2days, sats 45. I had given ventilator support. She had thrombocytopenia and neutrophilic leucocytosis along with hyponatremia and hypokalemia. Our physician opined that it could be a case of leptospirosis or vasculitis.

Dengue NS1, igM igG negative, scrub typhus negative. What is the possibility? What could be the cause? Please suggest.
