Refractory asthma is a "difficult asthma"(DA), where symptoms recur despite maximal conventional treatments. Those in whom persistent symptoms result from a failure of basic asthma management are described as "severe therapy-resistant asthma" (STRA). The majority of children have difficult asthma because of a failure to get the basics of a management right. However, it is necessary to note that with the advanced understanding and technology the number of STRA patients has reduced.

This article highlights different modifiable factors that can be identified in a general pediatric clinic to manage the "difficult asthma" in pediatric patients.
