This patient had acute abdominal pain after a year-long bout of chronic abdominal pain. Following observations can be made from the attached radiograph: Free air has allowed us to observe the entire diaphragm. Normally the soft tissues in the peritoneal cavity does not provide contrasting density to outline the undersurface of the diaphragm. Free intraperitoneal air is always due to ruptured viscus which itself contains air.
