This is regarding a patient, who is a frequent flight traveller as part of his job, developing severe earache, block at the end of each landing and lasting for a couple of days. It sometimes becomes so severe, to the extent he developed eardrum rupture with bleeding ear after one travel and he has become sort of phobic for air travel. He is now on prophylaxis Xylometazoline nasal spray, instructed to apply just an hour before the flight and a second dose after an interval for longer flights.
A daily local nasal steroid spray also started the result of which is still awaited. Kindly advise regarding the option of Myringotomy and grommet for this patient, whether it is indicated or will it be beneficial as an individual case, if not clinically indicated as per guidelines.
