A 30-year-old male presented with a bullet injury to cervical spine with quadriplegia. Bullet injury received in celebratory firing in marriage in UP. X-Rays and CT showed bullet lodged C6/C7. MRI could not be performed due to the lodged bullet.
The decision was taken to attempt to remove the bullet through a minimally invasive approach to prevent any further damage to the spine. Medtronic MetRx tubular retractor system was utilised to make a channel under fluoroscopy guidance. The bullet was successfully removed and muscle patch was placed to seal the dural tear. Post-op patient is recovering well with grade 1 power and recovery of sensations on the 5 th post-op day.
