A 16-year-old girl with uncontrolled focal seizures and reports 2 recent events in which she experienced "spells of confusion," "loss of vision," and altered responsiveness during and immediately following these events. She is afraid of falling or walking into dangerous situations, although this has never happened. Her last convulsion lasted approximately 10 minutes and she was hospitalized 6 months prior to this consultation. Her present antiepileptic drugs (AEDs) include oxcarbazepine 600 mg twice daily and topiramate 500 mg per day . These medications have decreased the number of events, but she is not seizure free.

She is shy, however, and interacts with stilted speech and behavior that is seemingly younger than that of a 16-year-old. Is her case a drug resistant epileptic case or a case of drug abuse can be suspected? What will be the next step to assess her condition? *This case…
