Linagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor has been approved as the second-line antidiabetic therapy and as first-line agents in people who did not tolerate metformin. Linagliptin improves insulin secretion, reduces glucagon production with added advantages of reduced risk of hypoglycemia, weight neutrality, and good safety profile. Linagliptin is excreted mainly via the enterohepatic system and hence suitable for patients with renal impairment without dose adjustment. These additional advantages of linagliptin make it a desirable antidiabetic for patients with cardiac and renal diseases.
