For fissure in ano it is advised to do a lateral sphincterotomy.This causes an addl.wound at operation site.I do a posterior spincterotomy thru' the fissure itself where the internal sphincter is already exposed.It is supposed to cause anal incontinence.I have not experienced any incontinence in my patients over last 25 yrs.Can anyone enlighten me in this matter?
