A 57-year-old man presented with a pain in the right knee with a recent onset of warmth and swelling in the right thigh. Examination Physical examination revealed an asymmetric increase in the right thigh volume with a normal range of joint motion. Radiographs (panel A and B) showed bony enlargement and cortical thickening (white arrow), along with coarsening of trabeculae in the right ilium and right femur extending up to the subchondral region (panel A and B, black arrows). Thickening of bilateral Ilio-pectineal and Ilio-ischial lines was also observed. The alkaline phosphatase level (69 U/L) was within normal range but bone-specific alkaline phosphatase level was 32µg/liter (normal range <20 µg/liter).
