A 63-year-old lady presented with episodic chest pain without specific triggers (no relation with exercise). Physical examination revealed no abnormalities. The ECG is shown in the attached image. Coronary angiography revealed a normal echo and coronary arteries. Her family history revealed no coronary artery disease, sudden death, epilepsia, or syncope. The patient did not have a syncopal episode before.

The cause of chest pain could not be identified. Coronary artery disease seemed unlikely. The abnormalities on the ECG persisted during the admission period, which lasted a couple of days. Can you suggest the diagnosis and management steps for this patient? *This patient case is from Docplexus Editorial Team for educative purposes only.
