A patient diagnosed to have extensive tinea corporis infection, asking for advice to a doctor at his private chamber. He has h/o application of steroid combination cream and itch guard by own from local medicine shop for 2 months. The patient is apparently normal & no past h/o DM, HTN, Thyroid dysfunction (no doctor, he never tested), no concomitant h/o medicine use on regular basis. Now is it be wise enough for the clinician to start without topical treatment & advice him to do blood report of CH, LFT, Urea, Creatinine, FBS, PPBS & KOH mount + culture sensitivity? & start Oral therapy on follow up visit after 4 weeks.if he did all d reports in its very much ok.

& if not DN give a note & start treatment. Is it makes a clinician safer? Rather DN starting oral therapy without knowing his LFT status & confirming the diagnosis by KOH or culture. But we know its not possible for every tinea…
