Re-admission is when a person is admitted to the same or another acute care facility within 30 days of an initial hospital stay. Re-admissions have a high financial impact, creating an economic burden on the patients. The most common reasons for re-admission include congestive heart failure, septicemia, and pneumonia. Some re-admissions can be prevented through standard quality ambulatory care several weeks or months before admission. Check out this infographic on the steps to prevent hospital re-admissions.
