35Y/F Non-hypertensive, Non-diabetic presented to opd vit C/O mild Swelling in the neck, Non-tender, No dysphagia, No change in voice, No cervical lymphadenopaty .No Symptoms of either Hypothyroidsm or Hyperthyroidism. On Investigations found FT3 2.75 (1.4- 4.4) FT4 0.90 (0.8-1.8) and TSH 6.77 (0.45-4.5). USG neck done. S/O 1) Enlarged both lobes of thyroid & Isthmus show Heterogeneous Echotexuture & increased Vascularity S/O Thyroiditis. 2) Few Hypoechoic Nodules in Rt. Lobe of Thyroid. Done Thyroid Antibodies Anti-TPO(AMA)> 600 & ATG IS 4000.
Suggested FNAC also, Reports are awaiting. I suspect that it could be subclinical autoimmune hypothyroidism & planning to start her vit Thyroxine (75ucg) as her weight is 50kg. Should I Consider any further test like ESR or anything? Is there any change in the plan of treatment needed? Please Suggest.
