60 yr old mild htn male presented with lt LMN facial palsy Rt plantar ext without weakness and left a hemicranial headache with Lt neck pain BP 140/80, CT scan BIL BG small lacunar infarct, no brain stem infarct noted, pt mild lethargic, no past history except multiple OTC self-medication. Localisation and suggest management.
