Case presentation A 70-year-old man presented with a one-week history of high-grade fevers, night sweats, and non-itchy rash associated with a burning sensation on his back, shoulders, and torso (Cover image). He also reported a one-month-long history of intermittent watery diarrhea (4–5 times daily), hematochezia, and mild abdominal discomfort. He denied recent gastrointestinal or upper respiratory tract infections, vaccinations, malignancy, myalgias, arthralgia, or headaches.

He was febrile (39.5°C) and tachycardic (heart rate: 115 bpm) on presentation, with normal blood pressure. The patient was a nonsmoker and did not consume alcohol.
