A randomized clinical trial conducted across three community clinics in Texas evaluated a multidimensional diabetes management program delivered by community health workers (CHWs) among 257 low-income, uninsured White Hispanic adults with type 2 diabetes. The intervention combined group diabetes education, individualized telehealth coaching, and a structured participant-CHW-clinician feedback loop designed to improve communication, address patient concerns, and enhance care coordination. After 12 months, participants receiving the intervention showed greater reductions in HbA1c levels (net difference, โ1.0 percentage point), total cholesterol (โ35.4 mg/dL), and LDL cholesterol (โ29.7 mg/dL) compared with those receiving usual care.
