A 25-year-old female presented with the two-year history of low mood, fatigue, anhedonia, shifting pains at different sites like head, abdomen etc, loss of appetite, sleep, dyspepsia, irritability, withdrawn behaviour, crying spells, palpitations. Referred by the physician for counselling. History of treatment with SSRI, BZD from different doctors including IP treatment in two tertiary centres. Now on Ayurvedic medicine. No specific stressors, family history of psychotic illness in a second-degree relative.

Old treatment record has USG abdomen done one and half year back which is within normal limits. Surgery consultation records indicate piles for which they advised surgery as optional. The detailed evaluation revealed persistent low Hb level since 2 years, significant weight loss, history of black coloured stool. CT abdomen revealed advanced CA stomach with extensive mets.
