A patient has Type 2 DM from the last 30 yrs. Current Scenario: Presently gradual wt loss.

The patient is taking pre Mixed insulin 50/50:14,14,12 and basal insulin 24 unit at night(glargine) Tab Glimepiride 4 mg, metformin 1000 mg, sitagliptin 100 Previously the HbA1C was 12.5, But now it is 8.9 Blood sugar fasting varying from 140 to 200 and pp 260 to 280 Body wt 55 kg GFR 55 Please suggest me further better management.
