Full term neonates with MSL develops tachypnea on day 2 documented upto 80-90 with no retraction after observing for few hours as it didn't settled hence shifted to Nicu for further care. In Nicu kept under O2 by hood bt as didn't settled hence put on cpap with mini setting of PEEP 4, Fio2 25%. 2d echo was s/o severe PPHN (50-60 mm Hg) with no other heart lesion. Started on oral sildenafil bt whenever taken off from cpap it again developed tachypnea.
Repeat 2d Ech on day 7 was s/o 40 mm hg. HRCT chest done was also normal. On day 12 x ray neck done to rule out upper airway obstruction was also normal; still d child on cpap and today tab Bosentan also added. So kindly plz give ur suggestion
