Perianal Crohn’s disease (PCD) is a severe and disabling manifestation of Crohn’s disease that often requires both medical and surgical intervention. Researchers conducted a multicenter retrospective study using the TriNetX US Collaborative Network to compare the effectiveness of ustekinumab (UST) and vedolizumab (VDZ) in adults with PCD following anti-TNF therapy failure. The study included 2,936 patients treated between January 2017 and May 2023, with propensity score matching creating two balanced cohorts of 936 patients each.
At one year, UST was associated with a 34% lower risk of definitive perianal fistula surgery and a 30% lower risk of composite surgical events compared with VDZ.
