I had a patient about 4 days back who came to me with the history of fever. He had seen one or 2 doctors and was also investigated. He was being treated as he had enteric fever. He showed me all the reports. His WBC count was high ESR was 45. I went into the detailed history when he mentioned that he had been treated for Pul. Kocks a few years back.

I examined him and asked for an X- Chest. I asked him in to continue Cephalexin which was stared for him till we get the X-ray report.Today I saw the report .It showed a old Kocks scar on the rt. apex and a new lesion on the left mid chest. Since he does not have any old papers. Can some one guide me as to the line of treatment to be started. I have also asked for sputum report for AFB.
