We know that medical records are to be preserved and a copy of the same has to be given to the patient when asked for. But which all are included in medical records. We have so many registers right from the autoclaving register to indoor admission registers to checklists which are used.

Does all that get included? Importantly is it mandatory to take a video recording of endoscopy and give it to the patient including unedited version or the edited version?
