A 28 years young man, brought for losing 11 kg weight in six months without any reason. The only explanation given by patient is that previously. He was single and posted outstation, eating out pizza, burger and I the hotel only and working late hours in office. Now transfered to home town, eating homemade food only. He lost 11.0 kg wt, but feels normal.
On investigations, positive findings are FT4=21.2 (22.0 normal), STSH 6.24, in LFT only albumin 4.7/globulin 3.0, in RFT only chloride is 92.0. Others are all tests are normal. USG only liver shows a fat-sparing area at porta hepatis. Now the query is weight loss and the fat sparing area should be suspected as HCC or metastatic lesion?
