What Is The Cause Of Pain At Rt Chest At Anterior Axillary Line At Level Of Hepatic Area?

A 21 year old woman came in OPD today, walking with pain at Rt lateral chest, leaning forward and to Rt side also. C/O swelling at Rt Hepatic area of CHEST wall, for two months with fever off and on taking treatment at various places without relief., unable to lie down on examination coach. On examination no swelling found, but marked tenderness at Rt Hepatic area with crackling sound on pressing and very severe pain.

TLC 13200/-- X-RAY CHEST PA VIEW shows Rt diaphragm higher up, usg shows minimal to mild PLEURAL Effusion, effusion fluid column is 13 mm in height in mid axillary line. In the poor socioeconomic conditions lady, what can be the cause of this pain? Look at the pictures also.
