A population-level study of 594,655 cesarean deliveries in California (2016–2021) revealed 10,182 cases (171 per 10,000) of severe perioperative surgical morbidity. Leading complications included wound problems (59/10,000), pelvic or bladder injury (45/10,000), ileus or bowel obstruction (33/10,000), and shock (15/10,000). Risk was notably higher for intrapartum cesareans (203/10,000) compared to prelabor cases (146/10,000), with placenta accreta spectrum disorder showing a 15.3-fold risk increase. These results highlight an urgent need to strengthen pre-op risk stratification, optimize surgical readiness, and reduce preventable morbidity.
