A 57 year old white male s/p double lung transplant for IPF comes to CTVS, lung transplant clinic with severe weight loss (15pounds in 1 month since transplant), anorexia, feeling unhappy, crampy ab pain, tremors, watches TV for long time, keeps the lights on and sleeping for hours in the bed, feel tired over gradual period of time. He denies c/o of SOB/Chest pain/cough or fever. He has been on Prednisone, mycophenolate mofetil, tacrolimus, acetaminophen, Magnesium, oxycodone, metoclopramide, no azoles. Had CMV mismatch from donor was treated. He is also taking Sertraline for severe grief bereavement after loss of his son 6 months ago.
Takes Metoprolol for HTN. Recent CXR after transplant-WNL, CT-Normal. Pre-transplant X-rays HRCT-showed IPF, underwent transplant a one and half month back now with similar unresolved complaints aforesaid without any improvement. He was put on following…
