Case Presentation A 60-year-old multiparous postmenopausal woman from rural Sudan was brought to the obstetrics and gynaecology emergency department in a wheelchair. She had been referred from a district hospital due to worsening pelvic symptoms. Chief Complaints Severe lower abdominal, pelvic, and perineal pain for four months Clear, odourless vaginal discharge for two months History of Present Illness The patient had previously been healthy, active, and fully independent in daily activities, including farm work.

Over four months, she developed progressively worsening pelvic and perineal pain.
