A 28-year-old man presented to the emergency department complaining of anal and lower-back pain for 2 days. Simple analgesics weren’t beneficial for this patient. The pain was progressively getting worse and finding it uncomfortable to walk or sit down. He is otherwise fit and well and smokes 10 cigarettes a day. Examination Inspection of the anus revealed a 3cm x 3 cm swelling at the anal margin.
