A year old man presents with profuse watery diarrhea and abdominal cramps. He has been admitted 1 month back for right sided inguinal hernia repair. Post-surgery he developed surgical site infection due to drug-resistant enterococci and was administered broad-spectrum antibiotics. The current complaint of diarrhea began 3 weeks after admission during which several antibiotics were administered to control the surgical site infections. On examination, patient is febrile, dehydrated and has generalised abdominal tenderness. The surgery site has healed with no signs of infection at present.

Investigations reveal leukocytosis, elevated serum creatinine, hypoalbuminemia and loss of electrolytes. Stool examination reveals pus cells and absence of parasites.
