A 55 years old male, with diabetes mellitus type 2 for past 15 years with bleeding right foot was rushed to our emergency ward. He noticed the bleeding when he woke up in the early morning to notice his mattress was red and the bleeding was on. It took him few hours to come to emergency ward as he lives in the rural area. The patient told that he has habit of examining his feet daily and he did not observe anything a night before. He was sure that bleeding occurred during the sleep.

The patient admitted a partial loss of sensation in the feet for last few months. On initial Examination patient was normal except bleeding foot and swelling. Patient did not have any ulceration, infection or amputation. But thorough examination showed that there was raw deep ulcer on a right big toe and serrated/eaten up edges. What can be reason for the bleeding/ eaten up edges of the wound?
