Abnormal uterine bleeding (AUB) is an underrecognized but clinically significant complication of oral anticoagulant therapy in women. Using a large U.S. national claims database, this nested matched case–control study evaluated the incidence and healthcare burden of AUB within one year of oral anticoagulant initiation among 276,911 users compared with 1,384,555 matched nonusers.
Anticoagulant use was associated with a significantly higher incidence of AUB (6.1% vs 3.0%; adjusted odds ratio [AOR] 1.81), along with markedly increased risks of AUB-related hospitalization, emergency department visits, and outpatient encounters. The relative impact was greatest in women younger than 50 years, who experienced nearly double the risk of AUB compared with age-matched controls, while older women also showed a significant increase.
