This 60 year old gentleman presented with Spreading Cellulitis in his Right Foot partially amputated elsewhere. He had severe rest pain in Right Foot. He is Diabetic only since 3 years and has never smoked. On examination he is moderately built. His lower limb pulses are absent bilaterally. Right Foot has only great toe.
The other toes have been amputated with a large infected ulcer. He was not willing for amputation of his leg and had even attempted suicides when he did not get proper treatments in his native place for many years. As he was obese and had many co-morbid factors including poor run off in the lower limb arteries, entering the abdomen and a conventional Aorto- Femoral Bypass was fraught with much danger and risks.
